| Abstract: | : | The emerging digital health initiatives have the potential to enable healthcare service providers to collaboratively and proactively manage population health, monitor disease status, provide at home acute care treatment, and maintain continuous communications with patients and caregivers. Digital health encompasses several different technologies – including Electronic Medical Records (EMR), patient portals, mobile applications, embedded sensor devices, iOT, and wearables. Collectively, current and emerging technologies offer amazingopportunities for patients and clinicians to work together to achieve improved health outcomes through coordination of care, health literacy, sharing of pertinent data and records, and continuous tracking of patient health indicators. The overall framework used in the talk discusses the impacts of digital health from four perspectives: Informing, Interacting, Monitoring and Educating. |
